IMPORTANT INFORMATION ABOUT YOUR LOAN APPLICATION
To help the government fight the funding of terrorism and money laundering activities, Federal law and Credit Union Policy requires all financial institutions to obtain, verify, and record information that identifies each person on a loan application and describe the purpose of the loan.
When you apply for a loan, we will ask for applicant(s):
name
mailing address
date of birth
other information
a brief description for the purpose of the loan
a copy of your most recent pay stub for income verification
If an applicant is not a credit union member, we will need a copy of their unexpired government issued photo identification such as a Driver's License, Military ID or Passport.
Applicant must provide the purpose of the loan.
Please provide two references: Name, address and phone number of nearest relative not living with you.
Signature Statement: By electronically submitting this application, you promise that everything you have stated in this application is correct and to the best of your knowledge. If there are changes you will notify us and you authorize the credit union to obtain credit reports in connection with this application for credit and any update, renewal or extension of the credit received. You understand the credit union will use this information to make its decision. It is a federal crime to willfully and deliberately provide incomplete or incorrect information on loan applications made to federal or state chartered credit unions insured by NCUA.
